This can exist in any form you like (document, presentation, film, audio etc.) but should include your evaluation findings and cover:
- What you did and who was involved
- Any changes made during the delivery of your project
- What difference the project made - the outcomes and experiences for those involved
- Case studies
- What you learned or reflected on
- How you might build on this project
A case study only needs to be two or three paragraphs and should highlight who the individual is, how they engaged with your project and benefited from it, and what has the impact been on them).
